Terms and Conditions

Dyslexia Welwyn 

 

Educational Screeners

By booking any educational screener with Dyslexia Welwyn you agree to the following terms and conditions:

Fees and Payment 

● The total cost of the screener is either £150

● The full cost of £150 must be paid no later than 14 days before the scheduled assessment. 

● If full payment is not received by this deadline, the original date may be released.

 

Assessment Outcomes 

● The report will include assessment data highlighting relative strengths and weaknesses in each subtest by including standard scores and/or percentile rankings alongside descriptive terms.

● The report will include include key recommendations

● The report will state whether further assessment or referrals are advised.

Report Process

● A 2 page report will be sent to parents/carers usually within 1 week of the assessment. 

● You will have 1 week to review the report and request amendments or clarifications.

● If you request any amendments a final report will be sent to parents/carers.

Sharing the Report 

● The final report is provided for your use and is your responsibility to share with any professionals (e.g. school staff, SENCOs, educational psychologists) who have a genuine reason to access it. 

● Reports are not shared, by myself, with third parties unless written consent is given.

It is the responsibility of the person booking the full diagnostic assessment to share any findings with those who have parental responsibility

By booking any educational assessment reports with Dyslexia Welwyn you agree to the following terms and conditions:

● The full cost of £250 must be paid no later than 14 days before the scheduled assessment. 

● If full payment is not received by this deadline, the original date may be released.

● The report will include assessment data highlighting relative strengths and weaknesses in each of the subtests by including standard scores and/or percentile rankings alongside descriptive terms.

● The report will include include full recommendations for both home and school

● The report will state whether further assessment or referrals are advised.

● A 4 page report will be sent to parents/carers usually within 2 weeks of the assessment. 

● You will have 1 week to review the report and request amendments or clarifications.

● If you request any amendments a final report will be sent to parents/carers.

 

Sharing the Report 

● The final report is provided for your use and is your responsibility to share with any professionals (e.g. school staff, SENCOs, educational psychologists) who have a genuine reason to access it. 

● Reports are not shared, by myself, with third parties unless written consent is given.

Visual Perception and Motor Coordination Assessment

By booking the visual perception and motor coordination assessment with Dyslexia Welwyn you agree to the following terms and conditions:

● The full cost of £150 must be paid no later than 14 days before the scheduled assessment. 

● If full payment is not received by this deadline, the original date may be released.

Assessment Outcomes

Writing Assessment Reports

By booking any educational assessment reports with Dyslexia Welwyn you agree to the following terms and conditions:

 

Fees and Payment 

● The total cost of the writing assessment is £200

● The full cost of £200 must be paid no later than 14 days before the scheduled assessment. 

● If full payment is not received by this deadline, the original date may be released.

 

Assessment Outcomes 

● The report will include assessment data, words per minute, descriptive analysis of their writing including any issues with motor control.

● The report will include include full recommendations for both home and school

● The report will state whether further assessment or referrals are advised.

 

Report Process

● A 3 page report will be sent to parents/carers usually within 2 weeks of the assessment. 

● You will have 1 week to review the report and request amendments or clarifications.

● If you request any amendments a final report will be sent to parents/carers.

Receptive Vocabulary

By booking a receptive vocabulary assessment with Dyslexia Welwyn you agree to the following terms and conditions:

Fees and Payment 

● The total cost of the screener is either £100

● The full cost of £100 must be paid no later than 14 days before the scheduled assessment. 

● If full payment is not received by this deadline, the original date may be released.

Assessment Outcomes 

● The report will include assessment data including the standard score alongside descriptive term.

● The report will include include key recommendations

● The report will state whether further assessment or referrals are advised.

● A 1 page report will be sent to parents/carers usually within 1 week of the assessment. 

● You will have 1 week to review the report and request amendments or clarifications.

● If you request any amendments a final report will be sent to parents/carers.

Full Diagnostic Assessments

By booking a full diagnostic dyslexia assessment, you agree to the following terms and conditions:

 

Fees and Payment 

● The total cost of the assessment is £500.

● A non-refundable deposit of £100 is required within 7 days of confirming the assessment date. 

● In addition to the deposit, a booking form must be completed and returned within 7 days of confirming the assessment date.

 ● The remaining balance of £400 must be paid no later than 7 days before the scheduled assessment. 

● If final payment is not received by this deadline, the original date may be released and a £100 rebooking fee will apply to secure a new date. 

 

Pre-Assessment Requirements 

● Questionnaires will be sent to both home and school settings. These must be completed and returned at least 7 days before the assessment date. 

● Information provided in these questionnaires may be included in the final report unless you request in writing that specific details be excluded.

● For students in Year 9 and above, please provide any completed Form 8s that detail current exam access arrangements. Please note that it is the responsibility of the school to assess and implement exam access arrangements; this does not fall within the remit of the assessor. 

 

Assessment Outcomes 

● A diagnosis of dyslexia will only be provided where the evidence clearly supports this conclusion. 

● In cases where the criteria are not met, the report will still include detailed findings and tailored recommendations. 

 

Report Process

● A 20+ page report will be sent to parents/carers usually within 3 weeks of the assessment. 

● You will have 2 weeks to review the report and request amendments or clarifications.

● If you request any amendments a final report will be sent to parents/carers.

 

Sharing the Report 

● The final report is provided for your use and is your responsibility to share with any professionals (e.g. school staff, SENCOs, educational psychologists) who have a genuine reason to access it. 

● Reports are not shared, by myself, with third parties unless written consent is given.

It is the responsibility of the person booking the full diagnostic assessment to share any findings with those who have parental responsibility.

School Support

● The school agree to pay the cost of £350 per day within 7 days of my visit. If the payment is late an additional charge of £50 will be added. The reduced cost of £300 will be offered to schools acknowledge by Dyslexia Welwyn as being in Welwyn Garden City/Welwyn/Hatfield.

● School agree to have all parental consent in writing in advance of my visit to agree to any individual/groups consultations including any observations, assessments or named discussions and written consent to share any reports or information with the school.  

● If school do not have the necessary parental consent in place, and the school have not cancelled the visit within 24 hours, then the full cost will still be charged. I will still visit the school and provide any unnamed support and advise and as required.
